Case Management Services
Helping adults and caregivers determine the client’s specific needs and locate available services to meet those needs.
The case manager often discusses federal or state-funded community-based services and other services the client can feasibly afford. Through case management services, persons in need of long-term care will discuss which daily activities they can complete independently, and those they can accomplish with some help. The case manager can then proceed to identify services that can help the client to continue living in their own home or community.
For instance, older persons recovering from a stroke may need home delivered meals or transportation service–or they may need help with medications, meal preparation or household chores. The case manager can help arrange these services in a package so that the client can continue living as independently as possible.
The case manager is key in developing a plan of care and linking the client and family caregivers with the appropriate agencies. They will periodically contact the client and family to determine if services need to be added or changed.
How We Deal with Long Term Needs
When long-term, intensive case management is needed, our experienced staff provides advocacy-based care management. The mission of this program is to provide an individualized care plan which reflects and preserves the client’s values and lifestyle choices, maximizes opportunities for positive change, and maintains the client’s quality of life.
